hello students this Mister Harris I'm your athletic director I'm here to give you some instructions on how to fill out the paperwork for athletics let me just say before we get started that are filling out the paperwork is important to not get on the field cancel all your paperwork is done appropriately and completely as long as it's not we can't we can't put you on the field later this year with all the problems with David so if your paperwork is not completed not perfect then I have to give it back to you till we get it filled out so nothing gets started until your paperwork is done properly so please pay close attention to these instruction of these will be helpful to you first of all when you go to fill out the paperwork you go to the school website looks like this you go to the athletic forms and documents and click on that what it looks like now these are all the four there aren't there are other information pieces here there's a checklist there's the links for K&K Insurance physicals for the clinic that I'll show you in a few minutes this is an athletic patient protocol which tells you what all the things that have to go on for you to be completely and turn in all of them are pretty self-explanatory the matter reading them and then signing them and turning them in and we'll make sure you pay attention so if it's for you to sign or your parents to sign up for both of you if it's if it's all those signatures we have to have all of them including yours in order for the paperwork to go through the first piece of paperwork though that I want to go through to make sure that you understand is the physical form looks like this so the physical form is two pages both pages it's work and need to be done the first page here's a checklist that you and your parents need to fill out and you and your parents need to do checklist and your parents need to fill out this top section your parents need to sign at the bottom second section is for your doctor when you go to the doctor to get your physical you will notice that there are things he also needs to sign it at the and he needs to stamp it is all of those things are your physical not be approached will not be approved and you will not be playing sports now if you the next full moon the next form that you need to fill out thunderstorms that's important will be athletic information and parent consent form that looks like no that's okay this is let me go back so this is the this is the emergency card so it says here eligibility emergency car so and it looks like this the emergency card is fairly easy and feel like you need to show you have insurance and if you don't have insurance you need to get a I'm going to show you an easy way to get some insurance a little bit later on but you need to fill out the insurance block when it's not filled out for the more I need a copy of your insurance card a copy of it a staple this form needs to be turned in in order for you to be approved okay the next form that I'm going to show you that is this this athletic information and parent consent for stye stop share and then I'm going to share again so that that form looks like this this is the form and it's there's a lot less versus top part of me and then their signature blocks for you and your parents here then your signature blocks down here again in order for this to work also there's this this warm on the back for the second that also needs to be signed if you do not do those forms if they're not. the next thing I want to show you is at the top of the of the thing that I showed you before this is the athletic links there's two things here to notice one is here is a Place West Oaks where you can get a physical today without an appointment with only a short for $20 they fill out the paperwork completely they'll give it to you so you know there's no need to wait for your physician to give you an appoint are you insurance to get you an appointment for $20 you can get your physical completely done perfectly and also Insurance Company insurance for the entire athletic here and it also only thought tossed $20 so these are it will send you there you give him the 20 bucks back information that you can print out and then include as your insurance card that will make you eligible so again these are all the pieces they were gone this year to something new a waiver of liability for covid-19 Dish think it's really easy there's a Spanish one and an English one do whichever one read it through and then sign and that's all all the rest of these forms are just super easy all you got to do is read them through and then sign them look for the signature blocks if there's a student signature block and there's only one of the other then only one of you needs the students please get this paperwork done quickly get it into us when it is finished you may turn it into the main office will be collected and sorted and then we'll get back to you about your eligibility Spanky LOL all I do is figure out how to stop this okay bye
