Athletic Clearance for all student athletes on a team, or trying out for a team.

 

STEP 1

All athletes will now get cleared through https://www.homecampus.com/select-state

Instructions located at the bottom of this page.

 

Click on "CA"

Create an Account

Start a new clearance - Make sure to select 'Valley Academy of Arts and Sciences" and the correct year (25-26)

Then complete ALL sections.

You will need to upload the physical, emergency card, and medical insurance card directly into your Athletic Clearance. If you do not have insurance come see the Athletic Director in the counseling office room B101.

 

Once you complete the athletic clearance you will get emailed a confirmation letter. - THIS DOES NOT CLEAR YOU. It simply states that this part of the process is complete.

 

STEP 2

 

Complete the Physical and Preparticipation Form - Click Me

 

TURN THEM IN DIRECTLY TO THE NURSE, OR ATHLETIC DIRECTOR - ORIGINALS ONLY

Preparticipation Form Checklist (Front):

• • Fill out completely AND explain all the yes answers in the space provided.
  • Make sure it is signed by both the parent/guardian and student-athlete.

Physical Form Checklist (Back):

• • Make sure the entire form is filled out and the doctor signs AND stamps the physical

Please ONLY give hard copies to the nurse or athletic director - emails will not be accepted

 

**Physicals are good for 1 year from the date of the physical.
